At the beginning of our sessions I will collect some personal data from you which will be stored confidentially and in line with GDPR regulations. This information enables me to run a safe and secure private practice, and will include:

Your full name, address, and contact details

Details of your GP

Details of your next of kin

This information will not be shared with any third parties, except in cases required by law, or in cases where you are at risk to yourself or others.
